Who Can Request a Mail Ballot?
Use our online Vote-by-Mail Request Form below to make a ballot request for yourself or an immediate family member. Our office will send your ballot wherever you need it to be.
Immediate family members include:

The deadline to request a ballot to be mailed to you is no later than 5 p.m. on the 12th day before Election Day.
Required Information
To request a Vote-by-Mail ballot, you must provide:

Full name of voter
-
Date of birth
-
Residence address
-
Mailing address, if different from home address (ballots cannot be forwarded)

Florida Driver License number, Florida ID number, or the last four digits of your Social Security Number

Signature of voter (written request only)
Your Vote-by-Mail ballot will include instructions on the proper way to mark and return your ballot. Read those instructions carefully.
